We provide respite care, assessment & rehabilitation support for older people who experience mental health problems who need support, care or protection. We work in partnership with Social services & the Community Mental Health Team.

How to access Havenhurst Care Home?
To access Havenhurst Care Home a person must have an assessment through the Community Mental Health Team (CMHT).

    The Respite care service can be booked throughout the year enabling the person & their carers to have a planned break.
  • Assessment &Rehabilitation 
    The Assessment & Rehabilitation service provides support to enable people to regain & maintain their independence as far as possible.

Havenhurst Care Home provides the following:

  • Healthy living & well -being support
  • Assist with individualised care needs
  • Various activities 
  • Nutritional & varied meals with dietary needs catered for.
  • Laundry

How much will it cost?
There may be a charge for staying in Havenhurst Care Home. People are asked to complete a financial assessment in order to determine ability to pay & how much will be charged for respite services. People who stay for a period of time on assessment will not be expected to contribute financially.

Hillside Residential Home

Hillside Residential Home provides a residential care service for adults mainly over 65 years old who need support, care rehabilitation or safeguarding including people leaving hospital; working in partnership with Social Services & Hywel Dda.

Hillside Residential Home is open 24 hours a day throughout the year & has respite, intermediate care & permanent beds.

How to access Hillside Residential home?

A person can access Hillside Residential Home for Respite or Permanent beds following an assessment of need & referral from Adult Care.

To access Hillside Residential Home for the intermediate care service the person will need to have a joint social care & health assessment.

What is the purpose of Hillside Residential Home?

 Hillside Residential Home provides a service that:

What does Hillside Residential Home provide?

Hillside Residential Home provides 3 types of services

  • Respite Care

The Respite care service can be booked throughout the year enabling the person & their carers to have a planned break.

  • Intermediate care

The intermediate care service provides rehabilitation therapy for up to 6 weeks to enable people to maintain their independence as far as possible. Where applicable this may be offered to people from 45years of age where the assessed need is identified.

  • Permanent Beds

The Permanent beds service provides a home for those in need.

  • Assist with individualised care needs
  • Various activities
  • Nutritional & varied meals with dietary needs catered for.
  • Laundry

How much will it cost?

There may be a charge for staying in Hillside Home. People are asked to complete a financial assessment in order to determine ability to pay & how much will be charged. This will be carried out as part of their assessment.

When are the Day Centres open?

The Day Centres are open Monday to Friday 9am - 3pm, except Bro Preseli in Crymych (Monday, Tuesday & Friday) throughout the year, excluding bank holidays & designated staff training days.

A person can access a Day Centre following an assessment & referral from Adult Care Management team.

What do the Day Centres provide?

The Day Centres provide a day service for older people living in the community who need extra support to enable them to maintain their independence in partnership with social care & health.

The purpose of Day Centres is to provide a service that:

  • Values & respects people as individuals
  • Promotes well-being
  • Supports & enables people to maximise independence
  • Supports people to make decisions about the services they receive
  • Allows carers to have valuable time away from their caring responsibilities

The Day Centres provides the following opportunities:

  • Healthy living & well -being
  • Activities including quizzes & board games
  • Gentle exercise
  • Lunchtime meal & refreshments
  • Assist with personal care, bathing & hairdressing
  • Arts & Crafts
  • Visits to places of interest

The majority of the activities are provided in house by the day centre staff with the exception of hairdressing & external activities which people pay for directly.

How much will it cost?

This depends on the outcome of a financial assessment in order to determine ability to pay. 

The day care charge for council owned Day Centres is £26.53 per day.

There is currently a maximum charge of £80 per week for all community based services.  

Lunches are charged at the current rate of £4.77 per day & there is an additional charge for activities.

Respite Care

Respite care is offered to our customers for a designated period of time (dependent on level of need) in an appropriate setting or via a direct payment. The aim is to provide a period of rest for the individual, and their carer, who may be finding it difficult to maintain their full independence at home, or in their care setting.

Respite is provided in various settings. These include:

  • Havenhurst / Milford House, Milford Haven - Mental Health & Elderly Mental Infirm customers
  • Residential Homes.

Milford House Centre

Milford House Centre provides services for customers with mental health problems who need support.

The centre includes:

  • Yr Hafan 24 hour supported accommodation consisting of 5 long stay beds
  • Yr Nyth – A 3 bedded staff support flat providing short term placements
  • Haven View Respite support in a 3 bedded flat/assessment unit
  • Dartmouth Centre – a day care centre for people with mental health problems
  • 59 St Peters Road a two bedroom satellite house with staff support
  • Trafalgar Road Project - a supporting people funded accommodation project with staff support to 6 single flats

Access to this service is through the mental health team assessment process.
